Absolutely! Rooted Pelvic Wellness will provide you a superbill upon request so you may submit to your insurance company.
A superbill is basically a receipt that includes your diagnosis code(s), treatment codes, billing units, and how much you paid. Reimbursement depends on your insurance and the out-of-network percentage.

This is a holistic approach, so I need to get to know you better before we begin any sort of physical assessment. I will ask you to share your medical and social history, your goals, and your daily activities & routines.
Depending on the reason for evaluation, we will usually move into a functional movement assessment to see how your joints & biomechanics could be reflecting the issues you’re having (or to improve upon if we’re working in a preventative model).
Following our external assessment, we can do an internal assessment if indicated and only with your explicit consent. We tell you every single step of what we’re doing with the help of our pelvic model, and help you connect to your pelvic floor!
At the end of the evaluation, we’ll go back to talking about your goals & come up with a plan of care to get you to those goals!
